What is Botox?

Botox is a procedure in which a drug called botulinum toxin is injected into a muscle to weaken that muscle's strength for several months.

Everyone knows the name, but let's start by clarifying it.

"Botox" was originally the name of a single product. Today it is widely used as a general term for botulinum toxin injections.

Botulinum toxin is a prescription-only drug (a drug that requires a doctor's prescription).

Botulinum toxin type A blocks the release of acetylcholine (the signaling substance that makes muscles move) at the neuromuscular junction (where a nerve passes its signal to a muscle). The injected muscle then contracts more weakly. Over time, new sprouts grow from the nerve terminal and temporarily carry the signal in its place, and then, as the original nerve terminal's function returns, the muscle's strength gradually recovers.

In simple terms — It is a drug that temporarily keeps the "move" signal from reaching the muscle. Over time the signal connects again, and the muscle moves as before.

What Botox treats

Botox (botulinum toxin) is an injection used to smooth expression lines that form when you smile, frown, or scowl. When the jaw muscles are overdeveloped and the face looks wide, it is also used to reduce muscle volume without surgery and slim the jawline.

It is mainly used for shallow wrinkles; for deep wrinkles, a method combining Botox with filling the sunken area with filler is sometimes applied. After injection the wrinkles smooth out gradually, and it takes about 2 weeks for the change to show most clearly.

Which wrinkles is Botox suited to?

It is considered for wrinkles caused by muscle movement, such as wrinkles that appear only when you smile or frown.

Even though they are all called "wrinkles," the reasons they form differ.

Wrinkles have several causes. Wrinkles that appear only when you make an expression are mainly a muscle issue. Fine wrinkles that are present even at rest are mainly a skin issue.

Deep wrinkles like creases are often a problem of the tissue beneath having sunken or dropped. What Botox treats is the first kind.

A Word from the Doctor

"It is not easy to think that the cause of forehead wrinkles lies in the eyes. Carefully identifying the cause through an accurate diagnosis is important."

As for the areas where we use Botox at our clinic (e.g., forehead, between the brows, eye corners, jaw muscles),

Face PlusBotox is recommended for people who

  • Want to improve wrinkles around the eyes, mouth, or forehead
  • Have developed jaw muscles that make the jaw look angular
  • Want to reduce unneeded muscle such as the trapezius
  • Want to prevent shallow wrinkles early
  • Want to improve nasolabial folds without surgery
  • Have developed calf muscles

What can't Botox resolve?

A sagging face, such as sagging cheeks and jawline, is not lifted by Botox, and hollow areas are not filled by Botox either.

So that expectations and reality do not diverge, we state clearly at the outset what Botox does not do.

Botox is a drug that reduces muscle movement. It does not lift loose tissue or add volume.

"Which product do you use?" is a question we often hear at consultations.

That does not mean one product is better than another. Products differ in the company that makes them, the manufacturing method, the amount in one vial, and the storage method.

Even if the unit used to measure the amount of drug has the same name across products, 1 unit of one product cannot be directly swapped for 1 unit of another.

As for which product is used in which case,

You are welcome to ask at your consultation which product you will receive.

05

How is the Botox procedure done? Does it hurt?

At the consultation we look at how your muscles move, decide the areas and amount, and then inject at several points with a fine needle.

Since it may feel vague the first time, we have laid it out step by step.

1. Consultation

We have you make facial expressions and look at which muscle is creating the wrinkle. If we judge that another method suits you better than Botox, we will tell you so.

2. Explanation and Consent

We explain the treatment areas and the reactions that may occur. If you are receiving it in an area that is not an approved one, we explain that as well.

3. Preparation

We mark the injection sites and disinfect them.

4. Injection

We inject in divided doses at the chosen sites with a fine needle.

5. Aftercare Instructions

We explain the precautions and the symptoms for which you should contact us.

How painful is Botox?

Most people feel a pinprick sensation, but it varies by area and by person.

06

If I get Botox often, will I develop resistance?

It is rare, but it can happen. If the body produces antibodies to the drug, the effect may weaken or disappear even with the same dose.

"I heard that if you get it often, it stops working later." This is a concern we often hear at consultations.

Side Effects

Repeated botulinum toxin treatment can cause some patients to develop neutralizing antibodies (antibodies that block the action of the drug). Neutralizing antibodies are found more often in patients whose response to treatment has decreased or disappeared (secondary non-response). However, about half of patients with secondary non-response have no neutralizing antibodies, so antibodies are not the only reason the effect has weakened. Factors related to antibody formation include a large amount received at one time and short intervals between treatments (including additional injections given at short intervals).

In plain terms — it is known that the more often and the more Botox you receive, the more likely the body is to produce “blocking antibodies” against the drug. Once antibodies form, the treatment may not work well. For this reason, when the effect fades, we do not immediately increase the amount or shorten the interval.

If the effect feels shorter or weaker than before, please tell us during your consultation. Together we will look at whether the cause is resistance, or the amount, area, or interval of the injections.

The standards we follow at the clinic to reduce resistance (minimum interval, amount per session, and when we change products) are

07

What should I be careful about after Botox?

It is best not to rub or press hard on the injected area. Detailed instructions are given after the treatment.

Here are the questions patients often ask about daily life after the injection.

  • It is best to avoid rubbing or pressing on the injected area.
  • On the day of the treatment, it is best to avoid saunas, jjimjilbang (Korean bathhouses), strenuous exercise, and alcohol.
  • If bruising appears, please apply a cold compress for the first few days.

Can I have Botox together with other procedures or surgery?

It depends on the type of procedure or surgery. Some are done on the same day, and others are scheduled on separate days.

If you are planning another procedure or surgery, you may first wonder how to schedule the dates.

Removing Forehead Wrinkles: Will Forehead Botox Solve It?

— Dr. Park Jung Keun, Plastic Surgery Specialist · Face Plus Talk
Female model touching her chin

When people think of forehead wrinkle removal, the first thing that comes to mind is forehead Botox. It is the simplest method. As you know, forehead Botox is simple to perform and involves little recovery burden. Of course, it is not permanent, so repeat treatments are needed, but because of the short recovery time it is a treatment worth considering first.

Forehead Botox does clearly make forehead wrinkles disappear. However, there are also many cases where the desired result is not achieved. In some cases the result can be very good, while in others the cosmetic result may not be good.

For example, let us look at an elderly man or woman whose eyelid skin sags a lot. The eyelid skin sags much more than you might think. When they simply open their eyes, the eyelid covers the pupil and blocks much of their vision. In many cases they can hardly see ahead. Opening the eyes can even feel heavy. So in order to open their eyes, they inevitably use every resource available. They strain the forehead with all their might to help open the eyes. That is how forehead wrinkles form, and the habit of constantly raising the forehead develops. Only then do they manage to open their eyes and secure their field of vision. Even so, opening the eyes remains tiring.

But if forehead Botox is given at this point to forcibly stop the forehead from working... the forehead wrinkles will disappear, but opening the eyes becomes even heavier, and sometimes the eyelids cover the field of vision, which can feel stifling. It is difficult functionally, and cosmetically as well, because the forehead can no longer be raised, the eyebrows drop and look unattractive.

Therefore, in such cases, the decision to have forehead Botox must be made carefully. Forehead wrinkles may need to be treated by other methods. Does this apply only to older people? Many young patients also have noticeable forehead wrinkles caused by sagging upper eyelid skin. Young patients should also be cautious about forehead Botox in these cases.

No matter how simple a procedure is, it does not apply to every case. The most important things are an accurate diagnosis and plan. It is the procedure itself that is simple; the result is by no means automatically natural. If the cause is accurately identified and the plan is made appropriately, a good result can be obtained. What is visible appears on the forehead, but the cause must be carefully sought elsewhere.

Frequently Asked Questions

When does Botox take effect, and how long does it last?

Usually the muscle's strength begins to decrease after a few days, and it gradually returns to the way it was after a few months.

When the change begins after the injection and how long it lasts are what everyone wants to know most.

ItemDetails
When change begins to be felt
Duration
Interval before the next injection

When the effect appears and how long it lasts differ by person, by area, and by the amount injected. For this reason, we do not promise a particular period.

What are the side effects of Botox? When should I contact the clinic?

Bruising, swelling, asymmetry, drooping eyelid, and an awkward facial expression can occur, and in rare cases systemic symptoms such as difficulty swallowing or breathing can occur.

This is information you must know before receiving the injection, so we have listed it as it is, without leaving out even the rare events.

Reactions commonly seen after injection

  • Bruising
  • Swelling
  • Redness at the injection site, tenderness when pressed
  • Headache

Reactions caused by the drug acting on muscles it was not meant to affect

  • Left-right asymmetry
  • Awkward facial expression
  • Drooping eyelid (ptosis, weakening of the muscle that opens the eye) — forehead, glabella, and eye-corner areas
  • Dry eyes — eye-corner area
  • Drooping eyebrow — forehead area
  • Drooping corners of the mouth or an awkward smile — area around the mouth
  • Weakness when chewing — jaw muscle area

These reactions usually go away as the action of the drug wears off. However, it may take several weeks for them to go away.

Conditions that may require treatment

  • Hypersensitivity reaction (allergic reaction)
  • Infection at the injection site
  • The action of the drug spreading from the injection site to a distant area (rare)

Do not wait in these cases

If anything feels wrong after the injection, please do not endure it alone.

The changes below are ones you should tell us about without waiting until your next visit.

  • When swallowing is difficult, your speech is slurred, or you are short of breath
  • When your voice is hoarse, or you have difficulty holding urine
  • When your whole body feels weak
  • When you see two images of an object or your vision is blurred
  • When your eyelid droops and it is hard to open your eyes
  • When your eyes stay dry or sore, or are sensitive to light
  • When the corners of your mouth droop, chewing is difficult, or your expression is noticeably awkward or pulled to one side
  • When you develop hives or itching, or your lips or the area around your eyes swell
  • When the injection site becomes red, swollen, and warm, or you have a fever
  • When bruising or swelling does not go down and gets worse

If you have difficulty breathing or swallowing, do not wait, regardless of clinic hours; call 119 or go to an emergency room.

If you develop hives all over your body, are short of breath, or your lips or throat swell, call 119 or go to an emergency room.

Will Botox lift a sagging face?

A sagging face, such as sagging cheeks and jawline, is not lifted by Botox. Botox is a drug that reduces muscle movement. Sagging is explained in How to Choose a Lifting Treatment.

Does resistance develop if I keep getting Botox?

It is rare, but it can happen. It is known to occur more readily the more often and the more you receive. If you feel the effect has lessened, we first look at the cause at your consultation before increasing the dose.
