Because this is surgery on the bone, you may have many worries. Below we explain in order the method, how it relates to square jaw surgery and chin surgery, recovery, side effects, and symptoms to have checked promptly.

01

What is V-Line Surgery?

V-Line Surgery is the name for planning and performing square jaw surgery and chin surgery together.

In a word, it is surgery in which the corner of the jaw and the chin are planned together at once rather than looked at separately.

Square jaw surgery works on the corner of the jaw below the ear, and chin surgery works on the very front of the jaw. If the two surgeries are planned separately, the balance of the jawline can be thrown off along the side of the jaw between the corner and the chin, so the method of looking at and planning the entire jawline at once, from the square jaw to the chin, is called V-Line Surgery.

Mechanism

V-Line Surgery is a method in which two surgeries, square jaw surgery and chin surgery, are planned and performed together. In the square jaw stage, mandibular angle resection (cutting the corner of the lower jawbone) cuts the corner in a curve to reduce the angle in the profile, and lateral cortex resection (thinning away the outer bone plate on the side of the jaw) reduces the width from the front. In the chin surgery stage, a method of narrowing a wide chin (removing the middle segment and bringing the two sides together and fixing them) is also used. V-Line Surgery connects the two surgeries within a single plan and refines them so that the border fits smoothly from the corner of the jaw to the chin.

In plain terms — V-Line Surgery is planning square jaw surgery and chin surgery at once and performing them together.

  • From the corner below the ear to the side of the jaw: refined with the square jaw surgery method.
  • A wide chin: narrowed with the chin surgery method.
  • The border between the two parts: refined so that they connect smoothly.

At our clinic, in the square jaw stage the basic method is cutting in a long curve from the corner of the jaw to the side of the jaw toward the front, with the outer bone plate (lateral cortex) added as a supplement if needed, and in the chin surgery stage we mainly use a T-shaped osteotomy, which allows the width, length, and front-back position to be adjusted together, with fixation by a metal plate or fixation pins.

The chin stage of V-Line Surgery is surgery that cuts the chin bone. It is a different surgery from surgery that places an implant in the chin.

A Word from the Doctor

"Simply reducing the bone is not what matters. How much is appropriately left is also very important."

The face is not flat

Facial contouring surgery must consider not only the size seen from the front but also the cross-section of the face, the volume of the sides, and dimension. We plan with the thought that, for the patient, it is a one-time-only chance, aiming for a natural facial line matched to ideal proportions and angles. Before surgery, the positions of the bones and nerves are checked on X-ray, and the surgical method is decided to fit each person's face.

Face PlusTo those who are worried about facial contouring surgery...

By what criteria do you choose a clinic? How can you minimize side effects, get a satisfying result, and avoid major problems?

From considering surgery and looking into it with interest to finally deciding to have it, a great deal of deliberation is needed. Even choosing a clinic alone involves too many options. And even after choosing a clinic, various worries arise about side effects, surgical results, and so on.

Seeing the people who visit the clinic for consultations or surgery, I have come to know the concerns that they commonly share. If you were ignored or brushed off after surgery, where could you even go to voice your complaint?

These are, in a way, natural expectations and hopes, but in reality, because we hear stories to the contrary through various channels, it is not easy to feel such confidence. It is good to visit several clinics, gather information, and compare. What is unfortunate is that you gain too much information but get buried in a flood of it, which makes judgment even harder.

Listening to people who have had consultations at several clinics, there are so many who are confused because they have a lot of information and knowledge about surgery but find it hard to judge what is important and what is not. In fact, many of them have consulted doctors who perform the surgery often and do it well, and in most cases what they were told is not very different.

The most important thing is to get a satisfying result while minimizing side effects. It is also to hope that if a problem does arise, it will be resolved smoothly.

Face PlusT-Osteotomy vs. Y-Osteotomy vs. Inverted-V Osteotomy: Which Is the Better Surgical Method?

In consultations, many people ask which of the various surgical methods, such as T, Y, and inverted-V osteotomies, is the better one. The answer is that 'the most suitable surgical method for you should be chosen after analyzing the anatomical structure through X-ray and CT.'

Face Plus does not perform osteotomy with one method only. Depending on the position and condition of the course of the patient's nerve line, we choose and carry out the most suitable of various methods.

Position of the nerve line — The nerve line runs from the back of the head to the hole in the lower jaw, but it is not a straight line; it hangs in a drooping curve, so the osteotomy position must be lower than the actual hole in the lower jaw

Position of the nerve line — The nerve line runs from the back of the head to the hole in the lower jaw, but it is not a straight line; it hangs in a drooping curve, so the osteotomy position must be lower than the actual hole in the lower jaw

The reason there are various surgical methods for chin surgery is that the position of the nerve line and each person's condition differ from person to person. The nerve line runs from the back of the head to the hole in the lower jaw, but the line is not straight; it hangs in a drooping curve, so the osteotomy position must be lower than the actual hole in the lower jaw. In addition, the position of this hole differs from patient to patient, the degree to which the nerve line droops differs, and the length and width of the jaw all differ, so the osteotomy method for chin surgery is decided by considering these points from many angles.

02

When is V-Line Surgery done?

It is done when not only the corner of the jaw but also the side of the jaw and the chin are wide, so the whole lower face looks square.

Female model with her hand under her chin

As you read the list below, picture your jawline in the mirror.

  • When, from the front, the jaw looks widely flared from below the ear to the jaw beside the corner of the mouth
  • When, even if the corner of the jaw is reduced, the chin is wide and angular and the jawline seems likely to look dull
  • When you previously had surgery on the corner of the jaw only, and the area where it joins the jawline toward the front looks stepped

We do not recommend surgery right away just because something applies. The first of our treatment principles is "We never recommend excessive surgery." If the corner of the jaw alone or the chin alone is enough, we will tell you so.

A Word from the Doctor

"It is better not to think of doing every contouring surgery to make the face smaller. Your face may become smaller, but it may not become prettier."

Face PlusFacePlus Facial Contouring Is Recommended for These People

  • When the skeleton of the face is large when seen from the front
  • When the angle below the ear is angular when seen from the side
  • When lasers or procedures alone do not improve the contour
  • When the face looks wide and spread out
  • When the face is asymmetric
  • When the contour of the bone itself needs to be improved
03

Are there cases where V-Line Surgery may not be suitable?

If the jaw looks wide because of muscle, soft tissue, or sagging rather than bone, V-Line Surgery may not produce the change you expect.

This is why we first confirm the cause before deciding on surgery.

When the width is caused by muscle or soft tissue

If the corner of the jaw rises firmly when you clench your back teeth, the chewing muscles may be larger than the bone.

You may want to check in front of a mirror. The exact cause is determined at the examination.

At our clinic, we often recommend first reducing the volume with Botox injections rather than surgery that directly cuts away the masseter.

Which is larger, the bone, the muscle, or the soft tissue, is determined by an examination and a CT scan (a test that images the bone in three dimensions). Injections that reduce muscle are explained in Botox treatment information.

A Word from the Doctor

"Bone surgery is not the only kind of contouring."

When the jawline has started to sag in your 40s and 50s

If the jawline has become blurred because of sagging, reducing the bone can make the sagging more noticeable, so surgery to lift the sagging is considered first.

If a square jaw that has bothered you for years is now starting to appear together with sagging, please read the information below first.

Mechanism

The mandibular angle and the lower border of the mandible (the lower edge of the lower jawbone) form the skeleton that supports the soft tissue (skin, fat, and muscle) of the cheeks and jawline. When bone is cut away, the support is reduced, and the masseter and soft tissue detached from the bone for the resection may sink downward. The chin bone is the front support from which the soft tissue under the chin and at the front of the neck hangs, so if the chin is narrowed, moved backward, or shortened, the soft tissue under the chin may look loose when skin elasticity has declined. V-Line Surgery reduces both of these supports together.

In simple terms — The jawbone supports the cheek tissue and the tissue under the chin. If the bone along the whole jawline is reduced, the tissue above it may come down and the jawline may look more blurred instead. At an age when skin elasticity has declined, this change is easier to see.

That is why, in consultations with middle-aged patients, we look at the degree of sagging of the jawline and neck together with the shape of the jawbone. If the reason the jawline has become blurred is sagging, see the Facelift surgery information and the Neck lift surgery information first; if the area under the chin is loose, see the Double chin muscle tightening surgery information first.

When the teeth do not meet properly

If the lower teeth sit far behind or far in front of the upper teeth, V-Line Surgery alone may not be enough.

V-Line Surgery reduces only the outer contour of the jawbone and does not change how the upper and lower teeth fit together (occlusion). If the cause is the size or position of the whole lower jaw and a malocclusion (a condition in which the upper and lower teeth do not close properly) is also present, orthodontic treatment and corrective jaw surgery (such as double jaw surgery) must be considered together.

04

How is V-Line Surgery performed?

The usual method is to make incisions in the gum outside the lower molars and the gum inside the lower lip, then shape the bone from the jaw angle to the chin and close the incisions with sutures (stitches).

Incisions are made only inside the mouth. Dissolving sutures are used, and the stitches are removed inside the mouth after 2 weeks. Because the surgery is performed through the mouth, no scar is left on the outside of the face. In exchange, the bone is cut along a long line through a narrow incision inside the mouth, so it is important to confirm the position of the nerve in advance at the planning stage.

At our clinic, the basic method is to cut in one long curve from the jaw angle to the front of the side of the jaw, and if the width seen from the front needs to be reduced further, a plate of the outer bone (outer cortical bone) is added as a supplementary step. For the chin, we mainly use a T-shaped osteotomy, which allows width, length, and front-to-back position to be adjusted together, and the repositioned bone is fixed with small metal plates or fixation pins.

Surgery is performed under general anesthesia, and the operating time for square jaw surgery and chin surgery combined is roughly 2 to 3 hours. You stay in the hospital for one day after surgery.

V-Line Surgery combines square jaw surgery with surgery that cuts the chin bone, so it is performed under general anesthesia. It is a different operation from chin implant insertion, which is done under sedation or local anesthesia.

Tests for general anesthesia are done before surgery. An anesthesiology and pain medicine specialist administers the anesthesia. You fast for 8 hours before surgery. You may not drink even water.

Since it may feel vague the first time, we have laid it out step by step.

1. Consultation and planning

Using face photos and CT, we look at the shape of the jaw angle, the side of the jaw, and the chin, left-right differences, the route the nerve takes, and the position of the tooth roots. Please tell us about all medications you take and any conditions you have. Before surgery, tests for general anesthesia are performed. Even things that seem minor are fine to mention.

2. Anesthesia

An anesthesiology and pain medicine specialist administers general anesthesia. When you wake up, the surgery is already over.

3. An incision is made inside the mouth and the bone is cut from the jaw angle to the side of the jaw

An incision is made in the gum outside the lower molars, and the bone is cut in a long curve along the planned line from the jaw angle to the front of the side of the jaw. If the width seen from the front needs to be reduced further, a thin layer of the outer bone (outer cortical bone) on the side of the jaw is additionally removed.

4. The chin bone is shaped and fixed

An incision is made in the gum inside the lower lip, and the place where the nerve for chin sensation exits is identified and protected. If the chin is wide, a middle piece is removed, the bone is cut in a T shape, the two sides are brought together, and they are fixed with small metal plates or fixation pins. This step is surgery that cuts the chin bone, and no implant is inserted. The border is smoothed so that the line cut on the side of the jaw and the line cut at the chin connect smoothly.

5. Bleeding is stopped, the incisions are closed, and compression is applied

After the bleeding is stopped and the detached chin muscle is reattached in its original position, the incisions are closed with dissolving sutures. To reduce swelling and bleeding, a compression band that wraps the face and chin is worn for about 1 week. The stitches for both the square jaw step and the chin step are removed inside the mouth after 2 weeks. You stay in the hospital for one day after surgery.

How long does recovery from V-Line Surgery take?

Generally, you can return to some degree of daily life 1 to 2 weeks after surgery. Swelling lasts longer than this: much of the major swelling goes down during the first month, and the remaining swelling goes down slowly over several months. Sensation in the lower lip may take even longer to return.

If you have work or an important appointment coming up, this is probably what you most want to know. We have written it by time point.

Time pointCourse
First few daysSwelling is at its worst. Bruising may extend down to the neck, and the lower lip and chin feel dull.
1–2 weeksBy this time you can usually return to daily life to some extent. While the wound inside the mouth heals, you eat soft food
2 weeksThe stitches are removed inside the mouth
About 1 monthMore than half of the swelling goes down
Around 6 monthsMost of the swelling has gone down and the jawline is settling
Around 1 yearThe remaining swelling goes down a little more

The point at which you can return to daily life and the point at which most of the swelling has gone down are different. The speed of recovery differs from person to person, and the jawline immediately after surgery is not the final shape.

After surgery, you will visit the clinic 2–3 times over about 2 weeks. The number and dates of visits are adjusted according to the surgery and your recovery. Dissolving sutures are used for closure, and the stitches are removed inside the mouth after 2 weeks. Please keep the stitch removal day if possible; the other dates can be adjusted.

So that you do not have to endure recovery alone, the doctor and the nursing team check your swelling, pain, and condition together. The third of our treatment principles is “We see patients more frequently after surgery.”

Precautions after surgery

The key points are keeping the wounds inside the mouth clean and not putting excessive force on the jaw and chin.

We realize it may be inconvenient, but everything below is meant to help the wound heal well.

  • It is best to avoid chewing hard or tough food forcefully, or biting food off with the front teeth.
  • Be careful not to open your mouth wide or bite down hard for about 1 month.
  • It is best to avoid pressing on or bumping the jaw and chin.
  • It is best to avoid turning your lower lip inside out to look at the wound.
  • After meals, rinse your mouth to keep the wound clean.
  • Smoking slows wound healing and bone union.

Do not wait in these cases

The changes below are ones you should report to us without waiting until your next visit. If you have difficulty breathing, call 119 right away.

If something feels wrong after surgery, do not put up with it on your own.

Symptoms that call for an immediate call to 119 or a trip to the emergency room

  • Shortness of breath or difficulty breathing, or sudden swelling of the neck, under the tongue, or under the chin
  • When the tongue is lifted up or the neck swells so much that it is difficult to swallow saliva
  • When bleeding in your mouth continues and does not stop
  • Chest pain or sudden shortness of breath

Symptoms to have checked promptly

  • When one side of the jaw, the chin, or the neck suddenly swells and feels tight, and the pain gradually gets worse
  • When you have a fever, pus or a bad taste comes from the wound in your mouth, or the wound opens
  • When your expression differs between left and right, such as the corner of the mouth drooping on one side only
  • When the jaw or chin feels as if it is moving, when severe pain suddenly occurs while chewing, or when the way your teeth fit together feels different
  • When your lower front teeth become loose, sensitive, or discolored
  • When it becomes harder and harder to open your mouth
  • When one calf becomes swollen and painful

Changes you must tell us about at your next visit

  • When numbness in the lower lip or chin stays the same even after several months, or you develop a tingling pain
  • When a difference between the left and right jawlines, a newly formed angle below the jaw angle, or a step between the side of the jaw and the chin does not lessen over time
  • When the chin looks like it is sagging downward, or your lower teeth show much more than before
  • When you feel sagging below the jawline, in the cheeks, or under the chin

Outside clinic hours, if you have difficulty breathing or bleeding that does not stop, go to the nearest emergency room or call 119.

Face Plus Talk

Is a 3D CT really necessary for facial contouring surgery?

— Dr. Park Jung Keun, Plastic Surgery Specialist · Face Plus Talk
Dr. Park Jung Keun

Before facial contouring surgery, tests needed for the surgery are performed. The tests fall broadly into two groups. One is the set of tests needed for general anesthesia, and the other is the X-ray and 3D CT for the facial contouring surgery itself. Both are essential tests.

However, opinions can differ on whether a 3D CT is absolutely necessary for facial contouring surgery. The X-ray taken for facial contouring surgery is actually not an ordinary X-ray but the kind mainly taken in dentistry. There is a difference between a simple X-ray and a 3D CT.

In fact, from the surgeon's point of view, the more important test that must be available is the dental radiograph rather than the 3D CT. The reason we look at the images is not simply to see the overall shape. A major purpose is also to confirm where important structures run.

Of course, it is important to understand asymmetry, the overall shape of the bone, and the degree of severity. What matters even more is to understand in detail where the important structures are located so that surgery can be done without damaging them.

Now, in that sense, the 3D CT is more advanced, so it might seem that it must always be done, but in practice a simple dental radiograph is often more useful. To the general public, it seems that things are easier to see in 3D, but for the person actually performing the surgery, that 3D view is not a great help.

However, when some problem occurs, a 3D CT can be very helpful for identifying and dealing with a complication. In such cases, a 3D CT can often confirm things that are hard to confirm with a simple X-ray.

Patients sometimes bring up the 3D CT a lot during consultations, and online advertising about the 3D CT seems too exaggerated. Of course, it is good equipment and it is expensive equipment, but it is wrong to talk as if operating without a 3D CT were a big problem. There seem to be many such misunderstandings, so today I talked about the 3D CT.

There is no single correct answer for a surgical method. After a thorough consultation, we proceed with a surgical method tailored to each individual. Online Consultation

Frequently Asked Questions

When is revision V-Line Surgery considered?

For cosmetic purposes, revision surgery is mostly recommended 6 months or more after the operation, but depending on the cause, it is sometimes performed right away.

If you are bothered by the result, we fully understand the wish to fix it as soon as possible.

The timing of revision surgery depends on the cause. In cases like the following, it is better to operate right away without waiting.

  • When the repositioned chin bone is not fixed well
  • When the bone has not united and is moving
  • When there is a wide gap between the cut and repositioned bone segments
  • When there is inflammation or infection

Cosmetic revision surgery to refine the shape is mostly recommended 6 months or more after the operation. Before then, swelling remains and the bone and surrounding tissue are still settling into place, so what you see now is not the final shape.

Mechanism

In revision surgery, the scar tissue and adhesions (tissues sticking to each other) left by the first operation must be dissected again (separated between the layers). A revision to smooth a secondary angle or the step between the side of the jaw and the chin is possible, but bone that has already been cut away heavily, or bone that has been reduced close to the inferior alveolar nerve and mental nerve, is difficult to reduce further and also difficult to restore to its original size.

In simple terms — A site that has been operated on once has tissue stuck together, so it is harder than the first time. A newly formed angle or step can be smoothed, but bone that has been cut away too much is hard to fill in again. This is why the amount to be reduced in the first operation is decided carefully.

What about anesthesia, operating time, and hospital stay for V-Line Surgery?

An anesthesiology and pain medicine specialist administers general anesthesia, and when you wake up, the surgery is already over. Tests for general anesthesia are done before surgery, and you fast for 8 hours before surgery, without even water. The operating time for square jaw surgery and chin surgery combined is roughly 2 to 3 hours, and you stay in the hospital for a day after surgery. The incisions are made only inside the mouth, dissolving sutures are used, and the stitches are removed from inside the mouth after 2 weeks. Because V-Line Surgery combines square jaw surgery with surgery that cuts the chin bone, it is performed under general anesthesia and is a different surgery from chin implant placement, which is done under sedation or local anesthesia.

Does narrowing only the chin create a V-line?

If the chin is the only wide area, chin surgery alone may be enough. If the side and corners of the jaw are wide as well, V-Line Surgery, which combines chin surgery and square jaw surgery, balances the entire jawline. The chin surgery included in V-Line Surgery is surgery that cuts the chin bone, and it is a different surgery from chin implant placement.

After V-Line Surgery, when can I return to daily life, and when does the swelling go down?

You can usually return to daily life to some extent 1 to 2 weeks after surgery. Swelling lasts longer than that: most of the major swelling goes down during the first month, and the remaining swelling goes down slowly over several months. Sensation in the lower lip sometimes takes longer than that to return. Dissolving sutures are used for closure, and the stitches are removed from inside the mouth after 2 weeks.

What should I do if my lower lip stays numb after V-Line Surgery?

Decreased sensation in the lower lip and chin is a common change and is usually temporary, but in some cases it remains for a long time. If it stays the same even after several months, or you develop a tingling pain, please be sure to tell us at your visit.
